Exercise-Mimicking Pill ENV-308 Clears First Human Trial, But Doctors Say Weight Data Is Missing

Women's Health reports that ENV-308, an investigational oral drug designed to mimic a hormone the body releases during intense exercise, has passed its first clinical trial in humans. ENV-308 is built to imitate Lac-Phe, a signaling molecule the body produces during hard exercise and after meals. What the trial measured

The news is based on Phase 1 results in 88 healthy adults, a trial designed to assess safety and tolerability rather than effectiveness. The article reports that participants tolerated the medication "well," that researchers described an "exceptional" gastrointestinal safety profile, and that no participants had serious side effects or stopped taking the drug.

The trial also looked at leptin, an appetite-regulating hormone released by fat cells that is typically chronically elevated in people with obesity. The Phase 1 results found the pill reduced circulating leptin, but the article states the findings do not show that the drug causes weight loss or improves leptin sensitivity.

How it differs from GLP-1s

Christine Bonarrigo, PharmD, a weight management clinical pharmacist at Tufts Medical Center, told the outlet that GLP-1 medications such as semaglutide and tirzepatide act on incretin pathways regulating appetite, blood sugar and digestion, while ENV-308 targets the Lac-Phe pathway instead. Paul Sack, MD, an endocrinologist at MedStar Health, described the hormonal pathway as "very different."

Mir Ali, MD, of MemorialCare Surgical Weight Loss Center, said the drug seems more designed to complement GLP-1s or to support maintenance after weight loss than to replace them.

What is not known

Bonarrigo said there is no human data yet showing how much weight ENV-308 can help people lose. Sack said the muscle-mass effect has only been shown in animal studies, and the company's stated hopes about weight maintenance and muscle retention come from its own press release.

Outlook

Ali said it will likely be years before the medication reaches the general public, even if the remaining trials go well. Until then, the article says, exercise remains the way to get exercise-like results.
