Semaglutide Slowed Epigenetic Aging Markers in a Small Trial, But Researchers Urge Caution

A Nature Communications study covered by Yahoo reports the first evidence that semaglutide, the active ingredient in GLP-1 medications, may slow markers of biological aging in several major organs. The finding comes from a reanalysis of a past trial in adults with HIV-associated lipohypertrophy, a group that often shows accelerated biological aging even when the virus is controlled.

Semaglutide is sold as Ozempic and Wegovy and is prescribed for weight loss and type 2 diabetes. Biological aging here means the epigenetic kind: DNA methylation patterns, the chemical tags that switch genes on and off, which researchers read using tools called epigenetic clocks rather than a birth date.

Key results

Over 32 weeks, 45 participants received a once-weekly GLP-1 injection and 39 received a placebo injection. Compared with placebo, the semaglutide group showed slowed biological aging in blood cells and in the brain, heart, liver, and kidneys.

On one epigenetic clock, researchers estimated the pace of biological aging was slowed by 9 percent. A separate clock showed reduced expression of inflammatory markers that the authors link to age-related disease and early death.

A small pilot study pointed the same way. Among 41 people living with HIV and fatty liver disease taking a once-weekly one-milligram injection, researchers observed a slower rate of biological aging in 42 percent of participants and longer telomeres in nearly 49 percent.

Why inflammation is the suspect

The proposed mechanism is not exotic. People with diabetes taking GLP-1 medications have been found to have fewer inflammatory biomarkers, including CRP and TNF-alpha, and the study authors note that chronic immune activation is a key driver of accelerated aging in people with HIV.

Visceral fat is the other lever. GLP-1s lower the fat that wraps around internal organs, and an excess of it releases more pro-inflammatory signals.

Limitations

Lead author Michael Corley, of the UC San Diego School of Medicine, is blunt about the ceiling on this: "We are not saying that semaglutide reverses aging or makes people younger." What he describes instead is a signal that some biological processes associated with aging may be slowed.

Both trials were small and drawn from immunocompromised populations, so whether the same thing happens in healthy cells is unknown. A larger trial is needed to tell whether any effect is temporary or sustained.

Bottom line

This is a genuinely interesting result and a very narrow one, which is a combination the longevity market handles badly. On the source's own reckoning, a definitive answer on semaglutide as an anti-aging drug is a couple of years away.
